Daily Wellness Check

Daily Wellness Check graphic Before leaving for work or sending a student to school, conduct a wellness check to ensure good health. If you or anyone in your home is ill, please stay home. This tool is not meant to take the place of consultation with your healthcare provider or to diagnose or treat conditions. If you are in an emergency medical situation, call 911. The following daily wellness check is designed to assess whether you should come to school or not.

Daily Wellness Check

If you or your child, in the last 48 hours, has had any of the following symptoms:

  • Fever (100 degrees or +)
  • Chills
  • Cough
  • Headache
  • Muscle or body aches
  • Nausea, vomiting or diarrhea
  • New loss of taste or smell
  • New runny nose or congestion
  • Shortness of breath or difficulty breathing
  • Sore throat
  • Close contact with a person with
    COVID-19 in the last 14 days

PLEASE STAY HOME and contact your healthcare provider and school.

  • Students must be fever free for 24 hours without the use of fever reducing medications in order to be at school.
